My father went into Wexham Park walking and came home needing a hoist. Can that be turned round?
Often at least in part, and it is one of the most frequent reasons families ring. Bed rest costs an older person muscle astonishingly quickly — the research shows adults in their late sixties losing more lean muscle in ten days than a forty-year-old loses in a month. What went that fast can come back, though more slowly than it left. We would assess what is actually in the way, work at it properly and be honest with you about what we think is achievable, rather than promising the hoist will go.
My mother has stopped doing her own shopping and now has it delivered. Is that a reason to ring?
It can be, and it is worth an honest look. Delivery is sensible and convenient, so families rarely read it as a warning — but if the reason underneath is that she can no longer carry a bag from the car or manage the walk to the shop, then a useful piece of daily exercise has quietly gone. Nothing needs to have happened for this to be worth assessing. The work is far easier now than after a fall, and it is aimed at something she actually wants back rather than an abstract exercise.
My grandmother has not stood up for over a year. Is it too late even to try?
It is not too late to look, and looking honestly is the point. The longer somebody has not done something, the more impossible it feels — that is normal, and it is not evidence that nothing can change. What has usually been missing is time rather than potential: enough weeks to rebuild the strength and the confidence that standing needs. We would assess what is actually in the way and tell you plainly either way. We do not promise outcomes, and we have not yet met anybody with nothing at all to work with.

Slowly, and treating the confidence as a clinical problem rather than a personality trait. Most people can do considerably more than they believe, and a large part of our work is proving it in steps small enough not to frighten anybody. We do not arrive with a programme and put her through it. The first visit is mostly finding out what she has stopped doing and why, then choosing one thing she would like back. An hour of decent company does more for confidence than it sounds, and it is part of the treatment rather than a courtesy.
She has carers morning and night and she is still going downhill. Why?
Because the visits are not where most of her day happens. Two calls a day account for perhaps an hour; the other twenty-three are hers, and if nothing in them asks her to move, that is where the ground is lost. It is not a failing of the calls — they are doing what they were commissioned to do. What we add is something for the hours in between: two or three specific things she does on her own, at times that fit her day, and somebody checking whether they are actually happening rather than assuming.
What should we be doing between your visits without making things worse?
Less than you probably are, and we will show you exactly what. Usually it means letting her stand without a hand under the elbow, letting her walk to the kitchen rather than carrying the tea through, and giving her the ten extra seconds it takes to do something herself. The hardest part is not helping — watching somebody struggle briefly when you could step in is genuinely difficult, particularly for a daughter. We are clear about which struggles are safe and useful and which are not, so you are not left judging it alone.
