Coming home from hospital in a city of steps

The Royal Sussex County Hospital on Eastern Road is Brighton’s acute hospital and the region’s major trauma centre, part of University Hospitals Sussex. Rehabilitation and outpatient services for the city run largely through Brighton General on Elm Grove, headquarters of Sussex Community NHS Foundation Trust, which also runs a specialist falls and fracture assessment service.

The Royal Sussex uses Discharge to Assess on its elderly care wards. Rather than keeping someone on a ward while checks are carried out, the assessment happens at home, where the real obstacles are. It is the right way round — waiting on a ward makes people weaker, not stronger.

But it means the front door arrives quickly. A discharge assessment establishes whether someone is safe. It does not, on its own, get them back up their own staircase, or down the six steps to the street, or as far as the seafront.

Independence is not a care plan. It is getting to the toilet on your own, and making your own cup of tea in your own kitchen.

Care calls are timed and task-based, because they have to be. But when everything is done for you, the body quietly stops practising — and in a Brighton house that shows up fast, because almost every useful thing is on the other side of a step.

A hospital stay takes confidence as well as strength, and confidence only comes back by doing the thing. So we take positive, considered risks with you, and stay alongside you while you do it: getting to the toilet on your own, walking through to make yourself a hot drink, managing the steps down to the garden. Yes — it is one of the most common reasons families call us. The Royal Sussex County Hospital on Eastern Road is Brighton’s acute hospital, and its elderly care wards use Discharge to Assess, so people go home and are assessed there rather than waiting on a ward. That is the right way round. What it does not do on its own is get someone back up their own staircase, or down the front steps to the pavement. We pick up after the assessment and work on exactly those obstacles, in the actual house, week after week.

This is very common in Brighton, and it is usually the steps rather than the walking. Someone can move perfectly well from room to room and still be housebound, because six steps down to the street stand between them and everything else. We work on the steps themselves, on confidence, and on a goal that means something — for a lot of people that is getting as far as the seafront, where the promenade is flat and sheltered. Yes. We are a private service, so you do not need a GP or hospital referral and there is no waiting list. You call us directly and we arrange a first visit within days, usually at a time that suits the family as well as the patient. If you would prefer to explore statutory support first, Brighton & Hove City Council’s adult social care team is the place to start, and many families use both.

A great deal, and the earlier the better. Falls are rarely just bad luck — strength, balance, footwear, medication and the layout of the house all play a part, and most of those can be changed. We assess all of it in the home where the falls actually happen. Brighton General on Elm Grove, the headquarters of Sussex Community NHS Foundation Trust, runs a specialist falls and fracture service, and we are often working alongside that rather than instead of it.

Yes. Our Brighton cover runs the whole way along the coast — Portslade, Hangleton and Hove to the west, and Ovingdean, Rottingdean, Saltdean and Peacehaven to the east — as well as the city itself. Visits are £93, plus 55p per mile round trip from your physiotherapist’s base. If you are further out, towards Lewes or west of Portslade, call us anyway and we will tell you honestly whether we can reach you.

Yes, and we do it regularly. Families often arrange and pay for additional physiotherapy beyond what the home itself provides — a set number of visits after a fall or a hospital stay, or ongoing work to keep someone moving. Alzheimer’s Society — support for people living with dementia and their families across Brighton and Hove, including local groups and a dementia adviser service.

Hop 50+ — a community centre for over-fifties in Palmeira Square, running the Pit Stop and Hop Stop dementia groups alongside a wider programme of activities.

Brighton & Hove City Council — adult social care is the route into statutory assessment, equipment and care at home. Many families use council support and private physiotherapy side by side.

Brighton and Hove has a large independent care-home sector, alongside homes run by national providers such as Bupa, Hallmark, Country Court, Gold Care Homes and Maven Healthcare. Wherever your relative lives, we can visit them there.

Most of this work is arranged and paid for privately by families — a block of visits after a fall or a hospital stay, or ongoing work to keep someone moving.
